Three things. 1. Looks to me like everything worked fine there, the shell is NOT SUPPOSED to start until you connect the debugger, go back and read the article you said you are following (it states exactly how to do that).
2. If you don't want the debugger to pause and wait for you to connect set the "debugSuspend" property to false (it is true by default - http://gwt-maven.googlecode.com/svn/docs/maven-googlewebtoolkit2-plugin/debug-mojo.html#debugSuspend). 3. You DON"T need to add the debugger options in "extraJvmArgs" - that is redundant. The gwt:debug goal does that for you automatically.
