With regard to the menstruation discussion, it would seem that there are many factors to take into consideration. It's a long time since my degree but from what I can remember about fertility and family size in Early modern Europe, women's experience varied according to social status,(e.g.wet nursing = more births) environmental factors, ( eg. crop failure = poor nurition = fewer births) and of course locality.
