There is indication in some "Italian" portraits of the possibility of
petticoats in the late 1400s. The counter argument is that the bodice and
skirt are two different materials but sewn together. I am sure there are
more theories out there.
As for closures, there is the possibility of hook and eye but being that
most garments had a tendency to be made to fit most people (hand-me-downs),
I would think that the drawstring waist band would be the most common form
of closure for the petticoat/underskirt.
I am going under the assumption that it is the under skirts that are being
asked about as I vague remember that petticoat has changed in item
identification over the years.

Petticoats for 13th-15th century? I thought it was all one-piece garments up
to the 16th C or so?

In any case, for the late Middle-Ages, I'd say lacings mostly, and some
buttons maybe. But that's an awful long period to cover, and you don't say
where, so it's a little hard to be more specific.
