Gather the sleeves into wristbands, closed with ribbon ties. The neck gathers can be stitched onto a band or facing (something like the ecclesiastical surplice, only they usually have a wider band at the top than you would want on a shift). According to Nicole Kipar, who used to be on this list, the ribbon ties should not be stitched on but should pass through two buttonholes (for 17th century garments).
