It's hard to say.

The shaping is incredibly common in other portraits of the same time.

Hip padding was around at the time - so it could be that, continued over the 
belly.

But then again, it is perfectly possible that it's the painting just isn't 
very well drawn.

It's also quite likely that she would be pregnant (thank the lord I'm alive 
now!!).

It's also possible that she's trying to look as though she were pregnant - 
it's been fashionable more than once or twice.
