I have many friends in the industry. They all will gladly tell you that you have to make sure you have it in writing that you give final approval of the cover. It is an often overlooked part of the contract.
Most of the folks I know have a few artist friends on retainer specifically for book covers. Grant it these are few and far between authors that do this but not every knows that can have it written up in the contract.
