Also, we had a bit smaller issue some time ago when we were using 1.2 version (lobInDatabase == false) - if transaction involving inserting BLOB is rolled back, temp files are hanging in the lobs directory until database is shut down. From your description it looks like this one might be fixed too. I can check that as well.
