Themes can include the plugin hooks to supply blocks, and so they can produce blocks that are theme-specific. There isn't an easy way I can think of to activate them when the theme becomes active, but it's probably easy enough to create and insert a new one into an area upon theme activation if one of that type in that area doesn't already exist.
