Maybe we should just remove the extra user option, and
make this the definite default for all platforms where it
applies, since - AFAIU - this shouldn't be a user choice
anymore now but rather something platform/compiler dependent,
and set automatically by Harbour if needed.

HB_WIN32_IO is default now for all Windows builds until user
will not specify HB_WIN32_IO_OFF. So now HB_WIN32_IO_OFF can
be use by users who will want to create some specific build
which will use only C RTL handlers. Otherwise by default Win API
is used.

Yes, I know, but does anyone could realistically need
HB_WIN32_IO_OFF? If not, and if this is just there for historical
reasons, we might as well remove this option to lessen the number
of possible build variations (to test & maintain).
