Joseph, If the dermatology application is HL7 compliant then my suggestion is that you use an integration engine approach rather than writing point to point interfaces with each application you need to interface with. There are open source integration engines such as JEngine http://jengine.org/ or HAPI from the University Health Network here in Toronto:
