I think I may have found it in a parameters file 100.23 parameter definitions. There are parameters like ORPF WORK COPY HEADER and ORPF CHART COPY HEADER and the same for FOOTER, etc., and once I found them in this definition file, there was a format for them and some entries, one of which , in the VA DEMO, is **** WORK COPY **** which looks a lot like what you found in your edition. There is some information about it in the CPRS GUI Technical Manual on page 167. It looks like maybe the Clinical Coordinator has the "POWER" to set at least some of these parameters, of which there are MANY to do with all sorts of things.
I would think that in the hospital it would be important for people to know that pieces of paper that were lying around were not "offical orders", which is why I suspect this is done and is so difficult to undo.
