The best way that I have found to register a patient is to use the function I put on this site:
http://openforum.worldvista.org/~forum/index.php?title=Register_a_Patient Copy this function into a text editor. Make sure the labels (i.e. the entry points: EDITPT,A etc) have no space in front of them. Then save the file as TMGMISC.m in the directory that has other .m files. I don't know how you are configured, but often there are two different directories with *.m files. One with the VistA files, and another with user files. You would want to put this in the directory with your user files, but it will work in either file.
