There is an HL7 package included in VistA which can be used to support v2 interfaces (right now, 2.4), but VistA does not provide any direct support for HL7 v3. The package supports scheduling of messages for delivery and assignment of received messages to specific applications for processing, automatic generation of commit acks, and tools supporting generation of application acks. At the segment level, VistA HL7 currently builds and parses MSH segments for you, but beyond that, it provides an API allowing you to iterate through the message a segment at a time and another API allowing you to extract specific fields. There are some strange omissions, such as no API to handle escape processing in strings.
