Are you using the free download version of Cache from the Intersystems web site? And you are using the FOIA of VistA from the VA ftp site dated August 25? Once you confirm this, I will try to get to straightening out the instructions by loading it up and giving it a test drive.
The domain names can be set to anything you want them to be, including your own domain. The options give were sometimes in the files, sometimes made up for the instructions.
