John Clemens writes, "it is trivial to parse the message and store the data where you want it." To this I would like to add that it is non-trivial to match commercial HIS business practices to VistA business rules, a necessary requirement when processing inbound ADT and encounters etc. to VistA.

Vista's HL7 package supports both inbound and outbound messaging just
fine. There is no such thing as a 'generic miracle parser' for inbound
data. If you know your message structure coming in and you know your
local application and database it is trivial to parse the message and
store the data where you want it. All HL7 interfaces work pretty much
that way whether you are Vista or a commercial vendor. You can leverage
many of the existing interfaces developed within the VA for storing lab,
pharmacy, ADT, etc or you can use them as a basis for a customized
interface. If messages strictly adhere to the standard, you would pretty
much know what to expect in every message, but ONLY you would know where
you want to store your data. It is simpler than XML to implement, though
version three help keep things simple.
I apologize if I've misunderstood some of the previous comments but I am
a strong supporter of HL7 and the Vista implementation. Some of the
newer features however can only be found in the patch documentation and
I don't know what the rules are for sharing that information (if any).

Yes, but getting your hands on it isn't easy.  It's on web pages on VA's
intranet.

VOE should be able to "track" with VistA as long as it's patches are
kept up to date.

VistA is -currently- very well set up for -sending- HL7 messages for
clinical data since the current evolution of VistA is to insure that all
clinical and administrative data can be hosted in national clinical and
administrative repositories for VHA.  Conversely, the display of a
patient's aggregate health record through CPRS -will- be done (in test
this fall) through a common data service that retrieves the data from
the repositories.

It would not be much of a stretch to host a "repository" in VistA for
smaller enterprises.

I am not so interested at the moment in IFCAP type data, but with
Patient related data.

After some research it looks like the HL7 receiver / responder has been
implemented via the VistA HL7 package, but that the actual MUMPS code
defining messages to be sent / received for various processes has not
been written.

Sounds like it would be a big job to provide integration with VistA
patient data.

Any other caveats / problems / concerns re the integration issue I
should be aware of?
