Do you recall at what point in the instructions that came up, because I would 
like to insert the fix in the instructions where it will do the most good.

i had the same problems with the device file not being setup.  to fix this,
 you must set the $I field of the CONSOLE device.  at the VISTA> prompt type
 W $I. you should see something like |TRM|:|1234|.  now, using fileman (ie D
 Q^DI) set the $I field to |TRM| or whatever is in that spot...
