That would work, but there's another problem to be considered, too: Changing
the definition like this would (potentially or, in some case, quite
certainly) break VistA applications that depend on the station number.
Cameron(?) described the approach used in HL7, which to treat them as coded
elements, where the significance of a code depends on the code set (ICD9,
SNOMED, CPT, what have you) to which it belongs.
That may be useful for messaging but boith a temporary and a permanent
convention must be worked out. Cameron also commented on soeme of the
difficulties. A convention using the present Station number data element
and representation but with unique structure till a permanent convention
that will be accommodate within those concventions that will emerge from
the ONCHIT NHIN can be arrived at would take care of the immediate need.
You cant yet see waht the permanent conevention might be.
I think the direction this
is all going is integration into a larger framework of vocabularies and
terminology services. But, of course, architectural considerations such as
these don't solve the immediate problem for VistA.
If you want to maintain
"binary compatibility" with standard VistA and you want these applications to
be usable outside VHA, you're pretty much up a creek.
With a temp convention and involvement in the perman ent convention work
you are not really "up-the-creek".
