Perhaps Javier means the hierarchal grouping of a hospital; the Department of Internal Medicine might have a few 'Firms', eg a 'Dr Osler Firm', or a 'Monday Firm' or a 'Nephrology Section'. It's useful to have these groups so that administrators can see the stats for each group, and for heads of departments and heads of these groups to administer their group.
I don't know if this helps, or what Vista's capability is in this respect.
