We have 3 different hospitals on the same server. Our
problem is that the software is different for each. It does not have to be but
it is.
I think it would be best to have a namespace for each
practice and a namespace for the software. The software would be common but the
data would be visible on to the practice that entered it. There are
disadvantages to this setup such as transferring a patient. If you did this
approach you would have 4 cache.dat files for 3 practices on a single
server.

possible to run multiple medical practices on a single VistA database, ie a
single Cache.dat? I'm not talking about the connection requirements, let's
assume those are handled for the moment.

It
seems an obvious enough way to use VistA ...why have a separate copy in each
medical office? With a high speed internet connection one could
remote desktop to a central VistA site. A less than desireable
configuration would be to have independent VistA databases at the remote
site. Each would need to be patched and configured
separately.
A
better configuration would be a single VistA database shared by all. Some
separation of data will be necessary and I'd like some hardhats expert to point
me in the right direction (if this is possible at all) to configuring VistA so
1)
patient records are shareable if desired, but can be kept separate when
necessary.
2)
Doctors are able to see a pool of their own patient records but not the others
unless explicitly desired.

admit to being a bit perplexed by the VistA modules that purport to handle
centralized functions:
Patient Data Exchange is dated
1993
Network Health Exchange is dated 1996 - this allows the entire patient record to be transmitted. Still in use?
National Online Information Sharing is dated 1998
Master Patient Index is dated 2000 - uses HL7
Health Data & Informatics is dated 2005 - drug names assigned an ID

I am running a VistA demo installed in July '05 with Nancy
Anthracite's expert help. Thanks Nancy! The CPRS version is labeled
OR_30_187 and I'm
running Cache on Windows.
A technical question: How can I tell which of
the above modules if any - or of the 300 modules at http://www.va.gov/vdl for that matter - are included in the working copy of VistA I
have?
Is there a best place for me
to continue researching these issues?
