There are circumstances, such as an adult abusing a child, or threatening to harm himself or others, where the public safety has been deemed more important then maintaining an open venue of communication between a patient and a physician. In these cases, we can be held liable if we do not report.
There are also requirements to report infectious diseases, such as chlymadia (and I thought HIV also) to the health department. Physcians rarely do this themselves, but depend on the labs to do so. At least that is my understanding.
