One of the recent autoupdates to x64 did this same thing, although I'm
not sure what OS you're having this problem with. To fix, I rolled back
to earlier image and turned off updates for the time being.

have changed mouse, ps2 and usb
is on a KVM, have taken it off the KVM
have updated drivers both mouse and video ( do not know why I suspect video )
have disabled fast write
unconfirmed but when I disabled AV to install the video drivers it did not lock
up so I need to investigate that further ( AV is Symantec Corp 9.0 )
