The answer here is "It depends". The queue can be for both incoming and outgoing messages. The best way to look at your queue with sendmail is:

sendmail -bp

This will list each message, and the reason it is in the queue.

Generally, in sendmail if you have a large amount of messages in the queue, I would put the culprit at mainly spam messages being in the queue.

Are you using something like MailScanner or SpamAssassin to help process messages before they are delivered? What is your setup?

We recently had to switch away from sendmail due to the cumbersome nature of blacklist_recipients, etc.


Now running on a system of Postfix + MySQL + MailScanner + SpamAssassin + ClamAV and it is much smoother.
