Ubcd.  Run antivir, then superantivirus, then spybot.  Reboot. Install 
something like avast with a boot-time scan... Run spybot one more time, boot 
scan.  Done

Wipe out all temporary files using ubcd before you start scanning.

Yes, very frustrating application.

Malwarebyte Antimalware (from the same Bleeping Computer Site) seems to 
clean up the Antivirus 2009 variant without having to mess around with 
safe mode.  It's been a long time since I've found a spyware removal 
program that didn't require safe mode.

Might want to give that one a try as well =)
