Generally, when someone says "shot in HD" they mean it was show using
digital cameras of HD (or greater) resolution.  Otherwise, as others have
pointed out they use analog film cameras and then have to convert it to
digital.

Some of the newer high-end digital  cameras shoot up to 4k resolution
(4096x2160) at 30fps for a pretty amazing price:
