Address range checking refactor.
Cleanup and refactor address range checking into macros.
Fix bug in "dump mmu" causing addresses near 0xffffffff to go unreported.
Also includes a new "lsheap" command - but it is commented out.
Minor header cleanups in memory.h / memory.cpp
