Here is an excerpt from our Circ Policy.  We are a day school, however,
thought you'd like to see another example.


Lost or Damaged Materials

All library users are expected to cover the cost of replacing a lost or
damaged item.  Replacement cost for a lost book will be requested after
30 days and after a search by the student and the library staff has been
made.  It the lost item is found and returned in good condition, the
student will be reimbursed.

Check-out privileges are suspended and transcripts and/or report cards
withheld at the end of the year until the replacement cost of a lost or
damaged book has been received.

Replacement Costs:

Replacement cost for a lost item or an item that is damaged beyond
repair (water damage, defacement, pages torn out, etc.) is determined by
the actual cost of the item, if found in library records.  If the price
of an item or its replacement cannot be determined, the following
replacement costs, which include the cost of the item plus processing,
will apply:

Hardcover Book.........$20
Reference Book.........$40
Paperback Fiction......$10
Paperback Non-Fiction..$20
Oversized Art Book.....$50
Videotape..............$40
Periodical (Lost back issue)...$1

Damaged books that need minor in-house repair...$3
