Dear Paul,
 From my experience in the Legal Deposit Department of JNUL, books 
with no author's name and deletion of the name from the Haskamot is 
not a rare phenomenon. The reasons for keeping anonymity are all the 
reasons both Gita and Aviva mentioned. Another reason for anonymity 
in Haredic books may be an innovative or provocative opinion that is 
expressed in them, or taking the non-conformist side in a 
Haredi-social or halachic debate.
Sometimes the author himself or his publisher tells me the author's 
name when delivering the obligatory copies, but this specific book 
was not delivered in the JNUL. Try sent me the phone number that 
appeared in the book, even though it is an old number. If the same 
author had written another book in recent years, maybe I can find him 
according the phone number in the inner database of the Legal Deposit 
Department, which is not public searchable.
