Hi  all,
  I was wondering if you could help me find a prayer that a patron remembers as 
a child during Rosh Hashanah. She comes from a conservative background.

Here's the description from the patron:
The cantor I asked says it comes right after Unetana Tokef;  it's the one 
"Mankind, Mankind" and I remember the ending - "yesofo l'afar" but I can't 
remember the coming from dust part ("[something] m'afar" that comes before the 
ending).
