The poem is quoted in the ethical will of Rabbi Judah ben Saul Aben Tibbon, The 
Path of Good Men (where it's attributed to "Ben Mishlei"), but I couldn't find 
it my Selected Poems of Yehuda HaLevi.  Here's the link to the ethical will:
