This patch fixes two issues with the OCC transactions:
- Overly conservative read-set. Previously a scanner was claiming read set on
the whole region. This patch gets the read set from the start and end keys that
a scanner was opened with (the start key comes from the arg to open, the end
key comes from a WhileMatch(StopRow()) filter if present.
- A potential ConcurrentModificationException.
