Looking at meta, was plain that a region was missing from .META. The install was 0.20.3 (0.20.4 should fix the issue seen but...). Getting things back online involved digging in logs to find info on missing region, what its encoded name was, and then doing a hack version of add_table.rb to add back the dropped region. A script that could spin through meta could easily find holes. That'd be a help. Subsequent, the script could go looking in fs for the missing region. If present, it could even add back the missing region.
