For clarity, the other-hemisphere matching is done by surface vertex index
(vertex 3287 of left hemisphere midthickness matches vertex 3287 of right
hemisphere), not by cifti indices.  To correctly translate between cifti
indices and surface vertex indices, you can use
-cifti-export-dense-mapping.  Any other method, like subtracting the number
of used left cortex vertices from the cifti indices, will be at least
subtly wrong, since we use different ROIs for the medial wall in each
hemisphere (which, as Matt says, means some cortex vertices in left
hemisphere may be considered medial wall vertices in the right hemisphere,
and thus the cifti file will not have rows for them on the other
hemisphere).
