Release Note: Previously, CallerContext was constructed by a builder. In
this new pattern, the constructor is private so that caller context will always
be constructed by a builder. (was: Currently CallerContext is constructed by a
builder. In this pattern, the constructor should be private so that caller
context should always be constructed by a builder. This is an omission of the
initial effort.)
