This does cause client operations to fail, and causes some applications to fail 
as well. Once a file gets in this state, recovery path back to a consistent 
state is not clear.
Do you have a test case or steps to reproduce this? Unfortunately, we don't 
have a corresponding exception trace at the Namenode.
