Nicholas, the issue about spec-ing case sensitivity of param names and values
in the query string has not been addressed.
This is a BIG issue to ensure interoperability.
If the spec states that the param names & values are case sensitive as per spec
I'm good. If the client/server implementation choses to accept case insensitive
it is fine, but it should always emit as per spec case.
