Looking at this a bit more closely, the FaultInjector in the test is going to 
try to cause an exception in the runnable task (StripedBlockReconstructor) and 
it catches all exceptions at the bottom of the run method. The actual task will 
not cause exceptions to appear in ErasureCodingWorker(...) even if it did throw 
an exception.

I am not sure if it is possible to create a test for this scenario easily - you 
need one of the earlier methods to throw a RunTimeException, as none of the 
methods are defined to throw a checked exception.

However I do agree that decrementing XmitsInProgress inside the exception 
handler in StripedBlockReconstructor is the wrong thing to do and we should fix 
it.

Have you actually seen this problem appear in a real cluster, with a 
RuntimeException logged here?
