Addressed all of the comments except for #3 above.

I agree wholeheartedly that #3 is a good idea, but when I went to do it, I 
faced the strange thing that we then had multiple methods in DFSUtil to create 
NN proxies. The old one, which doesn't do failover, is still referenced in a 
couple places. I think we should take care of this cleanup in (or maybe after) 
HDFS-2958. Does that sound reasonable?
