Details:
It would be beneficial to optimize the layout of the surgery form view.
The new main view will contain a notebook with the following pages:
* Risk Assessment
* Procedures
* Supplies
* Surgery team
The main procedure will be selected from the list of surgery procedures, and
it will be always visible in the main form.
The new view will be lighter and avoid scrolling.
