This was a correct guess. As of mainline,1403, the ia64 kernel booted on
a real machine is ticking reliably again. Unfortunately, there is still
some problem with UHCI, which is preventing me from controlling the
system using a USB keyboard.

So HelenOS found two UHCI hc's and two root hubs. uhcirh then attempted
to gain control of the hubs. Then I see a series of 6 warnings of the
following kind:

[uhci] WARN: Endpoint 0:0 both was left behind, removing
[uhcirh] WARN: usb_hc_new_device_wrapper: Failed to unregister default pipe

Eventually, the last line of output is:

[uhcirh] ERROR: Port (0x00000000000647b0 - 0): Faile(-301) to add
device: Operation stalled.

Any clues? What should be the next debugging step?
