Regarding the file «/etc/config.scm», the «guix install» command doesn't manage it. As far as I understand, this file is the operating system configuration generated by Guix graphical installer.
The «guix install» command is an alias for the «guix package --install» command, you can see what happens when it is run in section 5.2 Invoking ‘guix package’ of the Guix manual.
For what it's worth, I use a Guix System myself and never touch «/etc/config.scm». When I first installed the system, I copied it to another location in my home folder and rename it «os.scm» (you can give it any name), and this is the file I edit whenever I want to change the system configuration to later apply the changes by running «sudo guix system reconfigure os.scm».
