I used:
mke2fs 1.14, 9-Jan-1999 for EXT2 FS 0.5b, 95/08/09
successfully too.

I don't know if it helps, but try make a GRUB disk from the latest
version. It allows you to easily examine what's on the filesystem. I found
this invaluable with lots and lots of partitions on the disk.
