Sounds like either your tape has gone bad, or your tape driver; the fact that old versions of 'tar' work suggest that it's not due to any change in 'tar'. I suggest copying the tape with 'dd' and inspecting its bytes. You'll probably need to get the blocksize write, as a 'dd' option.
