For me I think one of the key differences between heroku and other systems is that it isn't me doing the admin when things do go wrong. Sure things will go down from time to time but even if I used a managed server that I had root access to the chances are that I would have to get involved, or more likely you would have to pay someone to be around and monitor the system.
