For posterity's sake here's what I ended up doing.

# To get everything working on Heroku, I had to add this …
group :production do
    gem 'pg'
end
# to the Gemfile to tell it to use postgresql in production (Heroku) and
then run

bundle install --without production
heroku rake db:migrate


I think you could skip the production stuff (i.e. just put pg in the
Gemfile and do a bundle install) if you have postgresql on your development
machine.
