Well, I seem to have fixed the problem. It appears that any namespaces used
in the extractors should also be declared on the documents that the
extractor sees, even if the namespace is not used in the document itself.
If i declare an extractor with
            <instruction property="href" namespace="http://hippo.nl/cms/1.0";
xpath="string(//cms:app-info/cms:href)"/>

and a document
<dummy/>

it breaks. But making the document
<dummy xmlns:cms="http://hippo.nl/cms/1.0"/>

works.
Hmmm, I can imagine this is because of the way the extractor executes the
xpath query, but I would consider it a bug
