website urls which are mapped to repository urls are not always case
insensitive right? This depends on the database configuration of your
Hippo Repository. Normally, you would configure the DB as case
insensitive. I ran into problems once (specifically under Linux) when I
didn't do that.

Only the host has to be case insensitive but not the following path.
When you use Cocoon, some URLs may seem case-insensitive, but that's
only for matches that use wildcards and request a repository source
from that wildcard, like
<map:match pattern="**.html">
<map:generate src="repository://{1}.xml"/>
<map:serialize type="html"/>
</map:match>
