We keep all of ours in the lab. Much easier than having HR search their files for the documentation.

Institutions vary on this, but most times all employee files had to be with HR. Education, CEU's, certifications, licenses and transcripts, along with evaluations or disiplinary actions were all based there and nothing was to be taken out or copied. I've been at a few places recently where accessing of staff files was possible through the intranet - but only certain levels of supervision were able to actually access a part or all of them.
