Fixed hivedoc.xsl - should now construct all docs using xml.  Regenerating the 
hivedoc for Hivemind itself looks ok.

Attaching patch - this also includes a fix to the ConstructRegistry ant task so 
that it excludes already processed modules (by module id) - this could be 
solved using the classpath of the task, but this adds a check in the task 
itself.  The problem arises because the task classpath includes both the 
hivemind code in the target directory as well as the hivemind libraries copied 
into the ext-package directory.
