What causes it is probably that you haven't played all the maps up to that point and there's no Save/Restore information for some entities that the map expects you to already have. I don't remember Valve ever stating that this would be fixed. What I remember them saying was (roughly translated) "That's the way it is. The map is designed in such a way that it expects you to have played the maps that come before it in sequence.". Basically don't load up a single player from the console and expect everything to work properly, many times it won't. Perhaps you are talking about some other type of problem leading to the crash. You should be a little more specific and provide what it is that's causing the crash (error messages, debugger line numbers, etc.).
