I had a chaintech geforce 4 ti 4200 where the fansink died. I didn't know until I was playing in a game and got confetti textures. the T&L unit also went out.
check and make sure that your heatsink is attached, and the fan on it is rotating freely. I don't think hl2 is broken for all FX series cards, maybe yours is just broken. Trust me, premature failures are nothing new to the graphics card market. my geforce is more complicated than the p4 that runs it.
